SMS Consent and Enrollment

Mosaic 1618 LLC provides this public page so users, carriers and messaging-service reviewers can verify the voluntary SMS enrollment process used in the private Mosaic Collector App. The application itself is available only to authorized users, so the exact consent presentation is reproduced below.
How a user opts in
- The authorized user signs in to the Mosaic Collector App.
- The user opens My Profile and enters or confirms a mobile telephone number.
- The user reviews the SMS disclosure shown below and actively selects its separate checkbox. It is not selected by default.
- The application records the user identity, telephone number, date and time, consent source, disclosure version and consent status.
- The user may withdraw consent at any time by replying STOP or by turning SMS messages off in My Profile.
Disclosure shown to users
I agree to receive recurring informational SMS messages from Mosaic 1618 about my Mosaic account, security, service availability, support requests, and collector-app activity I request. These messages are not promotional and do not include marketing from affiliates or other companies. Message frequency may vary. Message and data rates may apply. Reply STOP to unsubscribe or HELP for assistance. SMS consent is optional and is not a condition of purchasing or using Mosaic. See our Terms & Conditions and Privacy Policy.

- Program name: Mosaic.
- Message frequency: Message frequency may vary.
- Charges: Message and data rates may apply.
- Opt out: Reply STOP to unsubscribe.
- Help: Reply HELP or contact [email protected].
- Re-enrollment: A previously unsubscribed user must return to My Profile and affirmatively opt in again; a telephone number is never silently re-enrolled.
